Vertigo Management

Vertigo Management

Vertigo is a distressing condition characterised by a false sensation of spinning or movement, often caused by disorders of the inner ear or vestibular system. It can significantly impact daily activities, balance, and quality of life. Dr. Vishal Vikas Nair provides comprehensive evaluation and evidence-based management of vertigo, addressing the underlying cause for lasting relief.

What is Vertigo?

Vertigo is not simply dizziness — it is a specific sensation of rotational movement when there is none. It arises from dysfunction in the inner ear (peripheral vertigo) or, less commonly, from the brain or central nervous system (central vertigo). Accurate diagnosis is essential to determine the appropriate treatment strategy.

Common Causes of Vertigo

  • Benign Paroxysmal Positional Vertigo (BPPV) — most common cause
  • Meniere's Disease (fluid imbalance in the inner ear)
  • Vestibular Neuritis or Labyrinthitis
  • Chronic Otitis Media with inner ear involvement
  • Perilymph Fistula
  • Superior Semicircular Canal Dehiscence
  • Acoustic Neuroma
  • Migraine-associated vertigo

Diagnostic Evaluation

  • Detailed clinical history and physical examination
  • Dix-Hallpike and Roll tests for BPPV
  • Videonystagmography (VNG)
  • Audiometry and tympanometry
  • Electrocochleography (ECoG) for Meniere's Disease
  • MRI of the brain and internal auditory canals
  • Vestibular Evoked Myogenic Potentials (VEMP)

Treatment Options

Canalith Repositioning Manoeuvres

The Epley manoeuvre and other repositioning techniques are highly effective for BPPV, providing immediate relief in most patients.

Vestibular Rehabilitation Therapy

Customised exercises to retrain the brain to compensate for inner ear imbalance, improving stability and reducing dizziness.

Medical Management

Medications including vestibular suppressants, diuretics (for Meniere's Disease), and anti-inflammatory agents are prescribed as appropriate.

Intratympanic Injections

Intratympanic steroid or gentamicin injections for refractory Meniere's Disease to control vertigo episodes.

Surgical Management

In selected cases, surgical options such as endolymphatic sac decompression or labyrinthectomy may be considered for medically refractory vertigo.

Symptoms of Vertigo

  • Spinning sensation (room or self spinning)
  • Loss of balance or unsteadiness
  • Nausea and vomiting
  • Abnormal eye movements (nystagmus)
  • Hearing loss or tinnitus (in Meniere's Disease)
  • Feeling of ear fullness
  • Difficulty walking or standing
